Post-Surgical Rehabilitation · Baltimore, MD

Post-surgical rehab that actually finishes the job your surgeon started.

Surgery fixes the structure. Rehab restores the function. If you've had a joint replacement, spinal procedure, or soft tissue repair and you're still not moving the way you expected, one-on-one physical therapy at Physica Medica is where that changes.

Why post-surgical PT matters

The surgery worked. The recovery is where most people stall.

After surgery, your body is dealing with more than just a healing incision. Scar tissue forms. Surrounding muscles shut down or compensate. Pain changes how you move, and those movement patterns stick around long after the pain fades. Standard rehab protocols often address the surgical site — but not the full picture of how your body is actually functioning.

At Physica Medica, post-surgical rehabilitation starts with a thorough movement assessment. We're looking at what the surgery was meant to fix, what your body has been doing to protect itself since, and where the gaps are between where you are now and where you need to be.

Every session is one-on-one with a Doctor of Physical Therapy. No aides. No rotating staff. The same clinician who evaluates you is the one treating you, every visit.

Why Level 2 matters for you Most clinics offering dry needling are practicing at Level 1 — surface trigger points, accessible muscles. Level 2 certification adds advanced training in deeper structures, complex regions, and integrated treatment planning. In practice, that means we can treat cases at depth and complexity other providers refer out.
What we treat

Surgeries we commonly treat in rehabilitation.

We work with patients recovering from joint replacements — hip, knee, and shoulder — as well as rotator cuff repairs, ACL reconstructions, spinal surgeries including fusions and decompressions, labral repairs, and general soft tissue procedures. If your surgery isn't listed, call us. The evaluation process is the same regardless of the procedure: we assess what's there, not just what's on the surgical report.

What to expect

What Post-Surgical Rehab Looks Like at Physica Medica

The first session is an evaluation before it's anything else. We review your surgical history, imaging if you have it, and your surgeon's notes. Then we assess how you're actually moving — not just range of motion numbers, but the quality and compensation patterns behind them.

  1. 01

    Hands-on soft tissue work

Scar tissue and restricted fascia around a surgical site don't release on their own. We use manual therapy, IASTM, and where appropriate, dry needling to address tissue restrictions directly — restoring mobility before loading the area with strengthening work.

  2. 02

    Progressive movement retraining

Once mobility is there, we rebuild how you use it. Corrective exercise and movement retraining are built around your specific biomechanics — not a generic protocol handed to every post-op patient regardless of what their body is actually doing.

  3. 03

    Communication with your surgical team

If you have post-operative restrictions or a surgeon's protocol, we work within it. We can also communicate directly with your surgical team when coordination matters for your recovery.

  4. 04

A clear timeline, not an open-ended commitment

We'll give you an honest estimate of how many sessions to expect based on your procedure, how far out from surgery you are, and how your body responds in the first few visits. Recovery has a shape. We'll tell you what it looks like for your case.

  5. 05

What you may feel after early sessions

Some soreness after soft tissue work is normal, especially in the first few sessions. It's not a sign that something went wrong. We'll tell you what to expect before you leave, and how to tell the difference between productive post-treatment soreness and something worth flagging.

Who Post-Surgical Rehab at Physica Medica Is Built For

If you've finished hospital-based or clinic-based rehab and still aren't where you expected to be, that's a common reason patients find us. So is wanting one-on-one care from the start rather than a high-volume clinic setting.

Strong fit

  • Recent surgical patients cleared for outpatient physical therapy
  • Patients who completed initial rehab but plateaued before reaching full function
  • Anyone who had surgery months or years ago and is still dealing with stiffness, weakness, or pain they assumed was permanent
  • Patients who want a clear plan and a realistic timeline, not indefinite weekly appointments

Worth a conversation first

  • Very recent post-op patients still under specific surgical restrictions — we'll coordinate with your team
  • Complex cases involving infection, hardware complications, or revision surgery — we'll review and refer if needed
  • Pregnancy in certain regions — pelvic and low-back needling is restricted; other regions may still be appropriate
  • Genuine needle phobia we cannot work through — cupping, IASTM, or manual work may be a better fit
A note on cost & insurance

Physica Medica operates as an out-of-network practice. We provide a superbill after each session that you can submit to your insurance for potential reimbursement, depending on your out-of-network benefits. The one-on-one model is why results happen here — that's the trade-off, and most patients find it worth it.

Frequently Asked Questions About Post-Surgical Rehab

If your question isn't here, call us at 443-228-8029 or book a free consultation and ask directly.

Will insurance pay for dry needling?

Will my insurance cover post-surgical PT here? We're an out-of-network provider. We provide a superbill after each session that you submit to your insurer for potential reimbursement. How much you get back depends on your specific out-of-network benefits. We're happy to walk you through what to ask your insurance company before you book.

Who should not do dry needling?

How soon after surgery can I start? It depends on the procedure and your surgeon's clearance. Some patients start within days of surgery; others need to wait several weeks. If you're not sure, bring your post-op paperwork to the consultation and we'll tell you exactly where you stand.

How much does dry needling typically cost?

I already did PT at a hospital clinic. Why would this be different? High-volume clinic PT often means 30-minute sessions shared with an aide, a standard protocol, and limited time with an actual DPT. Here, every session is an hour, one-on-one, with the same clinician throughout your care. The treatment is built around your specific movement patterns — not a default post-op checklist.

Does dry needling hurt?

How many sessions will I need? It varies by procedure and how far out from surgery you are. We give you a realistic estimate after the first evaluation — not an open-ended plan designed to keep you coming indefinitely. Most post-surgical cases have a clear arc; we'll tell you what yours looks like.

How many sessions will I need?

My surgery was two years ago. Is it too late for rehab? No. Scar tissue, compensatory movement patterns, and muscle inhibition don't resolve on their own just because time has passed. Patients who've been told to 'just live with it' after surgery often respond well to targeted soft tissue work and movement retraining.

Is dry needling safe, and is the therapist certified in Maryland?

Do I need a referral? Maryland allows direct access to physical therapy — no referral required. If you have surgical records or imaging, bring them. If not, we work with what we find in the evaluation.
